HOW TO REGISTER YOUR ATHLETE

An athlete's parent/guardian must complete pre-registration online at Aktivate in order to participate in any BHMS athletic tryout process/athletic practice.

*ATHLETES MUST BE COMPLETELY REGISTERED ONLINE AND BOTH MEDICAL FORMS MUST BE COMPLETED & UPLOADED ONLINE PRIOR TO REGISTRATION DEADLINES TO TRYOUT!

AKTIVATE INSTRUCTIONS (formerly Register My Athlete (RMA))

If you already have an Aktivate/RMA parent account do not create a new account. 

  1. Go to aktivate.com.

  2. Login and follow steps to register your athlete(s) for a sport(s). (Note: You only need ONE account, even if you have children in more than one high school and/or middle school. Do Not create another account if you have used Register My Athlete in the past.)

  3. Medical forms* are good for 1 year, from the date the form was signed: (1) BH Medical History/Pre-Participation Medical History form to be completed by parent/guardian and (2) BH Pre-Participation Medical Exam/Pre- Participation Physical form to be completed by a physician.

If you never created an RMA/Aktivate parent account.

  1. Go to aktivate.com

  2. Click Login

  3. Click Create Account

  4. Fill in personal information account information (Note: This should be the parent information. You will be using the site as a parent.)

  5. Click Create Account

  6. Lastly, input the account Verification Code that you’ll receive via email to confirm your account (Note: You will need to open another tab (do not close your current tab) and find the verification email in your email inbox (it may take a few minutes to appear, so be patient). You can copy and paste the code into the pop-up or directly type into it.)

After you have an account.

  1. Go to aktivate.com

  2. Login

  3. Under the Parents header, select "Click here to start/complete athlete registrations"

  4. Click Start/Complete a Registration (upper left hand corner of the page)

  5. Click Start a New Registration (this is where you will enter all of your Athlete’s information)

  6. Follow the prompts to complete all requirements for your school’s registration

* Medical forms can be downloaded on Aktivate once you have started registration for your athlete(s) under Physical Documents and must be completed to tryouts or to participate.

1. The BH Medical History/Pre-Participation Medical History form to be completed by parent/guardian

2. BH Pre-Participation Medical Exam/Pre-Participation Physical form to be completed by a physician

 

Once both forms are complete you upload the completed documents on Aktivate. In the event that the site is down, you can also download the BH Medical History/Pre-Participation Medical History form and BH Pre-Participation Medical Exam/Pre-Participation Physical form here.
